Immunization Information

New Requirements for the 2018-19 School Year

  • All students entering grades 4, 6, 12 are required to have the Hepatitis A Vaccine

 

Not a new requirement, but as a reminder to parents who have students entering Kindergarten, grade 6 and grade 12.

Additional vaccines that are required for school entry.

  • Kindergarten: Dtap, Polio, Hepatitis B, MMR, Hepatitis A (2 doses)
  • Grade 6: Hepatitis A (2 doses), Meningococcal, Tdap
  • Grade 12: Hepatitis A (2 doses), 2nd Meningococcal (2 doses unless 1st dose was given after 16th birthday)
